Good day Gentlemen,

My query is in regard to having just discovered one axle locked in on my 2012 D22. Could have been like this for a month.

Just wondering what damage , if any, this would have caused.

I doubt it would have caused many problems. By engaging the hub, you're locking that hub to the axle and therefore turning the diff over, but without engaging 4WD there's nothing to put drive to the axle. Further (in your favour), because only one hub was engaged, even putting it in 4WD will cause the diff to transfer power to the unlocked axle instead.
